If you have already submitted an application, thank you.  Please be advised the window for completing applications for your 2007 Kan-ed Broadband Connectivity Subsidy closes on Friday, April 27, 2007.  For information on the purpose of the program and the process for completing an application see the message below originally sent on April 10, 2007.

Dear Kan-ed Member:
 
Kan-ed will issue broadband subsidy payments for the period of July 1, 2007 through June 30, 2008.  All Kan-ed members are eligible to apply.  Funds to subsidize broadband connectivity will be made available in the amount of the actual cost of Internet connectivity for the applicant organization up to $3,000.00.  Any funds received from the e-rate or rural health care programs for connectivity will reduce this amount.  As such, broadband subsidies will be allocated to a member school district, library, hospital, or post-secondary institution, and must be used for the following purpose:
 
Broadband Internet connection services, upgrade to broadband connectivity, or equipment to facilitate broadband Internet access to enhance member connectivity to the Kan-ed network and Kan-ed services.
 
Broadband subsidy applications will be accepted beginning April 10th, 2007 until April 30th, 2007.  You must be a Kan-ed member to apply.  The Kan-ed Broadband subsidy application is a web-based form and may be located by pointing your internet browser to https://surveys.ksu.edu/TS?offeringId=67160.  
 
With this announcement you will find a copy of the web-based subsidy application form in the attached file.  This is provided to assist you in gathering the required data in advance.  Please have this information ready when you complete the web-based application survey.  If you would like a copy of your completed application for your records, please print each page from your Internet browser as you complete it.  
 
Upon completion of the application survey you will be prompted to download and print the 3 page document acknowledging that your organization will adhere to the purpose of the program.  Read the document, sign in the required places and return the original to Kan-ed by fax at 785.296.7052 or by United States Postal Service or other commercial carrier:
